Standout Performance Limited: Waiver Agreement

This Waiver Agreement (“Agreement”) is entered into by the undersigned participant (or, where applicable, the parent or legal guardian of a participating minor) and Standout Performance Ltd. (“the Company”).

Standout Performance Ltd. is committed to providing a safe environment and will take all reasonable measures to help participants avoid injury. However, it is important to recognize and accept that participation in gymnastics, dance, movement classes, personal training, coaching, or any related activities inherently involves certain risks, including the risk of physical injury.

By signing below, I acknowledge, understand, and agree to the following:

• Acknowledgement of Risk:
  
  I acknowledge that I have been informed of and understand the risks associated with participation in any classes, activities, or events organized by Standout Performance Ltd. I recognize that these activities may involve physical exertion and carry the potential for injury, accident, or other harm. I freely and voluntarily choose to participate (or allow my child to participate) in these activities with full awareness of these risks.
  
  

• Release of Liability:
  
  I hereby release, discharge, and agree to indemnify and hold harmless Standout Performance Ltd., its owners, directors, officers, employees, teachers, coaches, trainers, agents, and faculty members (collectively, “Released Parties”) from any and all claims, demands, actions, or causes of action arising out of or in connection with any loss, damage, injury, or death that may be sustained by me or my child(ren), or to any property belonging to me or my child(ren).
  
  
• Waiver of Rights:
  
  I further waive, on behalf of myself, my heirs, assigns, and legal representatives, any right to make or bring any claim, lawsuit, or action against the Released Parties for any damage, loss, injury, or death as a result of participation in activities with Standout Performance Ltd.
  
  
• Responsibility to Communicate Health Conditions:
  
  I certify that I and/or my child(ren) are in good physical condition and have no medical or other conditions that would prevent safe participation in these activities. I understand it is my responsibility to notify instructors, coaches, or trainers of any health concerns or limitations prior to participating in any class, session, or event.
  
  
• Safe Conduct:
  
  I agree to conduct myself (and/or ensure my child(ren) conduct themselves) in a safe and responsible manner at all times and to act within personal physical limitations.
  
  
• Parental/Guardian Certification (If Applicable):
  
  If I am signing this Agreement on behalf of a minor child, I certify that I am the parent or legal guardian of the participant and have the full legal authority to execute this Agreement on their behalf.
  
  
• Binding Effect:
  
  My signature (including electronic signature) confirms that I have read, understood, and voluntarily accept the terms and conditions of this waiver and release of liability in its entirety.
