I voluntarily agree to assume all risks and responsibility for any injury or accident, which might occur to me or my child during any of the classes I (or my child) attend(s). I also exempt, release, and indemnify Standout Performance Ltd., its owners, teachers, coaches, trainers and/or faculty members from any, and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, my children, or property which may arise out of or in connection with participation in any classes or activities conducted by Standout Performance.

I further voluntarily agree to waive my rights and that of my heirs and assigns to hold Standout Performance Ltd., owners, teachers, coaches, trainers and/or faculty members liable for such damage, loss, injury, or death. I understand that I should be aware of my (or my child's) physical limitations and agree not to exceed them and I shall conduct all activities in a safe and careful manner. I ensure that I and/or my children are in good physical condition and have no medical reason or impairment that might prevent me (or them) from taking part in Standout Performance Ltd. Classes or private coaching. It is my responsibility to communicate to my/my child's instructor/coach/trainer about any health conditions.

If I am signing this waiver for my children, I certify that I am the parent or legal guardian and have the right to waive these rights. My e-signature is proof of my intention to execute a complete and unconditional waiver of all liability, either on behalf of my child or on my own behalf, to the fullest extent of the law.
